General Information

• The deceased must be a person of Thai nationality.
• A person reporting the death should be a family member or a relative of the deceased such as a child, father, mother, sibling or relative.
• A person reporting the death may apply for the Death Certificate in person at the Embassy or by mail.
• Issuance of a Death Certificate is free of charge with processing time of 7 working days.

Procedure Prior to the Lodging of Application

• Bring the Death Certificate issued by a Registrar Office to obtain an authentication from the Department of Foreign Affairs and Trade (DFAT) of Australia.

Applying in Person

• Completed Application of Legalisation Form and Death Certificate Form, which can be downloaded from the Embassy’s website. Fill out the deceased’s given name and surname in Thai and in English as appeared in the passport or Thai national ID card of the deceased.
• Original Death Certificate issued by a Registrar Office bearing the Seal of DFAT.
• Original and a copy of Passport and Thai National ID Card of the deceased certified by a person reporting the death.
• Original and a self-certified copy of Passport and Thai National ID Card of a person reporting the death.

Applying by Mail

• Completed Application of Legalisation Form and Death Certificate Form. Fill out the deceased’s given name and surname in Thai and in English as appeared in the passport or Thai national ID card of the deceased.
• A copy Death Certificate issued by a Registrar Office bearing the Seal of DFAT.
• A copy of Passport and Thai National ID Card of the deceased certified by a person reporting the death.
• A self-certified copy of Passport and Thai National ID Card of a person reporting the death.
• Registered A4-Sized Envelope with the sender’s address to be returned by post.
